I will make a record of the above matters, together with my examination findings (under the current data protection legislation, this is classified as special category data) which will allow me to compare my findings and track your progress.
The records will also include your name and contact details (under the current data protection legislation, this is classified as personal data), in case I need to contact you in relation to your appointment or any other matter relevant to your treatment.
I will keep the records in hard copy format for as long as is reasonably required to facilitate your ongoing treatment.
I have no intention of sharing your information with any other party (in the UK or another country) unless you specifically request me to do so or I am compelled to do so by a legal or public interest obligation.
I am obliged to inform you that your information will not be used for automated decision making or processing.
Under the current data protection legislation, given that the recording of the above information is integral to your care, it would be reasonable to rely upon the justification of legitimate interests to process your data, however I will seek your consent to process your data at the first consultation.
